Managing medications can be challenging, especially if you are taking multiple medicines or living with a chronic condition. As a Clinical Pharmacist with an MPhil in Pharmacology, I provide evidence-based medication consultations to help you understand your treatment and improve medication safety. During our consultation, I will carefully review your current medications, medical history, allergies, and laboratory reports (if available). I will assess your medication regimen for potential drug interactions, duplicate therapies, side effects, and opportunities to optimize treatment. I will explain the purpose of each medication, the correct dosage, timing, administration techniques, possible adverse effects, and precautions in simple, easy-to-understand language. I also provide guidance on medication adherence, chronic disease management, and lifestyle modifications related to your therapy and answer any medication-related questions you may have. This service is ideal for patients with diabetes, hypertension, heart disease, asthma, arthritis, thyroid disorders, kidney disease, neurological conditions, and other chronic illnesses, as well as anyone taking multiple medications or seeking a second opinion regarding their medication therapy. The consultation is conducted online and typically lasts 30–45 minutes Before the appointment, you will be asked to share your current prescriptions, medication list, and any relevant laboratory reports or medical records. After the consultation, you will receive personalized medication recommendations and can ask follow-up questions if clarification is needed. Please note that I do **not** diagnose medical conditions or prescribe prescription medications. My role is to optimize medication therapy and improve medication safety. If I identify any concerns that require medical evaluation, I will recommend that you consult your physician or the appropriate healthcare specialist.
